You can buy them from many places, particularly newsagents, and at some
train stations / ferry wharves that have manned ticketing counters, and you
can top them up with credit using the machines at most train stations as
well. See http://www.retailers.opal.com.au/

But if you're arriving at Sydney Airport, you can just buy it from the
ticket counter at the airport train station.

The cards work on buses, as well as Sydney Ferries and the light rail
(which is half-closed for construction during LCA, sadly).
